- 15 -20 patients per day

- Drive high-quality, safe, and cost-effective patient care in post-acute care communities
- Inpatient rounding in skilled nursing facility, inpatient rehabilitation facility, and long-term care hospital
- Work closely with hospice, palliative care, and home care agencies for safe patient transitions
- Ensure timely documentation and billing through our EMR (Gehrimed) meeting Medicare guidelines

**Compensation & Benefits**

- Compensation is a productivity-based model (Fee for Service). Expected compensation is estimated income range of $235,000 to $278,000 annually with no cap on productivity bonus income potential.
