This is a permanent position.

Opportunity Details:
Full-time Nurse Practitioner role in an established Internal Medicine / Primary Care practice
Provide comprehensive primary care services across multiple outpatient sites
Collaborate closely with physician leadership and practice operations leadership
Conduct patient assessments and develop individualized care plans
Diagnose and treat acute and chronic conditions, including medication management
Deliver preventive care services such as screenings, wellness visits, and patient education
Maintain accurate and compliant electronic medical records
Participate in quality improvement initiatives and team-based care coordination
Other duties as assigned

Full-time schedule

Requirements:
Master’s degree or higher from an accredited Nurse Practitioner program
Active New Hampshire Nurse Practitioner license (or eligibility)
DEA certification (or eligibility)
Strong commitment to high-quality, community-focused primary care
Ability to work collaboratively within a multidisciplinary team
