Job Description
**Location: Cooperstown, NY**
**Experienced Preferred**
• Join an established primary care team of physicians and advanced practice clinicians in an NCQA Patient-Centered Medical Home practice
• Outpatient practice, Monday–Friday, 8 AM – 5 PM
• Provide comprehensive medical care for patients aged 18 and above
• Team-based care delivery model with dedicated nursing and administrative staff
• Fully integrated EPIC inpatient and ambulatory EMR
• Administrative time prorated based on FTE; ramp-up schedule based on experience
• Preceptorship opportunities available
• On-call coverage during business hours on a rotating basis
Compensation & Incentives
• Salary range: $107,714 – $133,699
• Sign-on bonus up to $50,000
Qualifications
• National certification as a Nurse Practitioner or Physician Assistant
• NYS licensure, BLS, and DEA required
• Experience preferred
