Travel RN - Home Health Case Manager (Barre, VT) — $2,434/36-hr, ASAP, Flexible Caseload

We’re looking for a skilled and compassionate RN to join a home health team in Barre, VT on a contract/travel basis. This role is hands-on and independent—ideal for a nurse who enjoys working in the community, building relationships with patients and families, and coordinating care across disciplines.

• Pay: $2,434 per 36-hour week

What you’ll do

• Conduct in-home assessments and create individualized care plans based on patient needs.
• Deliver skilled nursing services such as wound care, medication management, and patient education.
• Monitor patient status, document changes, and revise care plans as appropriate.
• Coordinate with physicians, therapists, social workers, and caregivers to ensure seamless care.
• Provide teaching to patients and families about disease management, safety, and home-care routines.
• Accurately document assessments, interventions, and outcomes in a timely manner.
• Maintain compliance with agency, regulatory, and quality standards.

What we’re looking for

• Active RN license valid in the practicing state.
• Current BLS certification.
• Prior experience in home health, case management, or community nursing is preferred.
• Strong assessment skills, clinical judgment, and care coordination ability.
• Comfortable working independently and managing a caseload of home visits.
• Reliable transportation for travel to patient homes.
