Requirements:
- Active LPN/LVN license in the practicing state
- Experience in long-term care, skilled nursing, or assisted living preferred
- Strong clinical assessment, medication administration, documentation, and communication skills
- Ability to follow care plans, collaborate with RNs and interdisciplinary team, and provide compassionate caregiving
Responsibilities:
- Assist residents with activities of daily living and monitor health status
- Administer medications and treatments per orders and facility protocols, under RN or physician supervision
- Accurately document care, observations, and changes in condition; report significant changes to the healthcare team
- Maintain a safe, clean environment and follow infection control policies
- Provide emotional support and companionship to residents
