Requirements:
- Active LPN license valid in Michigan
- Minimum 1 year LTC or equivalent experience preferred
- Current BLS certification
- Strong interpersonal and organizational skills
Responsibilities:
- Provide direct nursing care under RN or physician supervision
- Administer medications and basic treatments per care plans
- Assist residents with activities of daily living and hygiene
- Observe, report, and document changes in resident condition
- Accurately document nursing interventions and outcomes
- Collaborate with the interdisciplinary care team to support resident-centered care
- Follow infection control and safety protocols
