Rate: $1,400 per 40-hour week

Minimum Requirements

  • Valid, active LPN license for the state of New Hampshire
  • BLS certification

Core Responsibilities

  • Provide direct nursing care under RN/physician direction: medication administration, wound care, vital signs.
  • Perform focused patient assessments (physical exam, history, medication review) and implement individualized care plans.
  • Administer and document prescribed medications and treatments accurately per protocols.
  • Assess, clean, dress, and monitor wounds/incisions for signs of infection or complications.
  • Monitor and record vital signs (BP, pulse, temperature, respirations) and notify appropriate staff of changes.
  • Maintain accurate patient documentation of condition changes, treatments, and medication administration.
  • Educate patients and families on diagnoses, medications, self-care, and preventive health measures.
  • Administer and monitor IV medications/fluids and maintain IV lines as permitted by facility policy.
  • Provide catheter care (insertion, maintenance, monitoring) and identify complications.
  • Collect specimens (blood, urine, sputum) for laboratory testing.
  • Collaborate with RNs, physicians, therapists, and support staff to coordinate patient care.
  • Assist with patient comfort, mobility, positioning, and pain management.
  • Respond to emergencies per facility protocols and provide basic life support as needed.
  • Follow infection control practices and proper PPE use.
  • Safely handle, store, and dispose of medications and medical supplies according to regulations.
  • Advocate for patient rights and needs; escalate concerns appropriately.
  • Supervise CNAs/UAPs when delegated, ensuring tasks are performed correctly.
  • Maintain competency through required education and training.
  • Perform other duties as assigned by management.
