Requirements:
- Active Registered Nurse (RN) license.
- Minimum 1–2 years of Long Term Care or Skilled Nursing experience preferred.
- Strong clinical, organizational, and leadership skills.
- BLS certification required.
- Ability to manage multiple residents and priorities effectively.
- Preferred Qualifications
- Experience with geriatric and chronic care populations.
- Knowledge of MDS documentation and care planning processes.
Responsibilities:
- Assess resident health status and monitor changes in condition.
- Administer medications, treatments, and IV therapy as ordered.
- Develop, implement, and evaluate individualized care plans.
- Coordinate care with physicians, therapists, and interdisciplinary teams.
- Supervise and delegate tasks to LPNs and CNAs as appropriate.
- Document nursing care accurately and in a timely manner.
- Educate residents and families on care plans and health management.
- Ensure compliance with safety, infection control, and regulatory standards
