Schedule: 07:00 AM – 07:30 PM
Rate: Local: $1,051/36 hours
Requirements:
- Active LPN license for the state of Vermont
- BLS certification
Core Responsibilities:
- Provide direct nursing care under RN or physician direction: medication administration, wound care, vital signs monitoring.
- Perform focused patient assessments (physical exam, history, medication review) and contribute to individualized care plans.
- Administer medications and treatments accurately, following protocols and documenting administration.
- Assess, clean, dress, and manage wounds/incisions; monitor for infection or complications.
- Monitor and record vital signs (BP, pulse, temperature, respirations) and changes in patient condition.
- Maintain accurate, timely patient documentation of assessments, interventions, and outcomes.
- Educate patients and families on diagnoses, medications, self-care, and discharge instructions.
- Administer and monitor IV medications/fluids; ensure proper IV line function and site care.
- Manage urinary catheters: insertion (where permitted), maintenance, and monitoring for complications.
- Collect specimens (blood, urine, sputum) for laboratory testing per policy.
- Collaborate with RNs, physicians, therapists, and support staff to coordinate care.
- Assist with patient comfort, mobility, positioning, and pain management.
- Respond to emergencies and provide basic life support per protocols.
- Adhere to infection control standards and proper use of PPE.
- Handle, store, and dispose of medications and supplies safely and according to regulations.
- Promote preventive care (vaccinations, screenings) and participate in health assessments.
- Advocate for patients’ rights and needs.
- Supervise CNAs or unlicensed assistive personnel when assigned.
- Complete other duties as assigned by management.
