Rate: Local: $1,051/36 hours

Requirements:

  • Active LPN license for the state of Vermont
  • BLS certification

Core Responsibilities:

  • Provide direct nursing care under RN or physician direction: medication administration, wound care, vital signs monitoring.
  • Perform focused patient assessments (physical exam, history, medication review) and contribute to individualized care plans.
  • Administer medications and treatments accurately, following protocols and documenting administration.
  • Assess, clean, dress, and manage wounds/incisions; monitor for infection or complications.
  • Monitor and record vital signs (BP, pulse, temperature, respirations) and changes in patient condition.
  • Maintain accurate, timely patient documentation of assessments, interventions, and outcomes.
  • Educate patients and families on diagnoses, medications, self-care, and discharge instructions.
  • Administer and monitor IV medications/fluids; ensure proper IV line function and site care.
  • Manage urinary catheters: insertion (where permitted), maintenance, and monitoring for complications.
  • Collect specimens (blood, urine, sputum) for laboratory testing per policy.
  • Collaborate with RNs, physicians, therapists, and support staff to coordinate care.
  • Assist with patient comfort, mobility, positioning, and pain management.
  • Respond to emergencies and provide basic life support per protocols.
  • Adhere to infection control standards and proper use of PPE.
  • Handle, store, and dispose of medications and supplies safely and according to regulations.
  • Promote preventive care (vaccinations, screenings) and participate in health assessments.
  • Advocate for patients’ rights and needs.
  • Supervise CNAs or unlicensed assistive personnel when assigned.
  • Complete other duties as assigned by management.
