Minimum requirements:

• Active, unencumbered LPN license for the state of Vermont
• BLS certification

Core responsibilities:

• Provide direct nursing care under RN/physician guidance (medication administration, wound care, vital signs).
• Perform patient assessments (physical exam, history, medication review) and contribute to care plans.
• Administer medications and treatments accurately per orders and protocols.
• Assess, clean, dress, and monitor wounds and surgical sites for complications.
• Monitor and document vital signs and changes in patient condition.
• Maintain accurate patient records of treatments, medications, and status changes.
• Educate patients and families on conditions, medications, and self-care.
• Administer and monitor IV fluids/medications; manage IV lines.
• Provide catheter care and monitor for related complications.
• Collect specimens (blood, urine, sputum) for lab testing.
• Collaborate with RNs, physicians, therapists, and support staff to coordinate care.
• Assist with patient mobility, positioning, comfort, and basic pain management.
• Respond to emergencies and provide basic life support per protocol.
• Adhere to infection control practices and PPE use.
• Handle, store, and dispose of medications and supplies according to policy.
• Promote preventive care (vaccinations, screenings) and participate in health assessments.
• Advocate for patient rights and needs.
• Supervise CNAs or other unlicensed assistive personnel when assigned.
• Complete other duties as assigned by management.
