Requirements:

• Valid, active Occupational Therapy state license
• CPR and BLS certification from AHA or Red Cross (with AED)

Key Responsibilities:

• Assess patients’ physical, cognitive, and environmental needs to identify limitations and set measurable goals
• Develop individualized treatment plans in collaboration with patients and the interdisciplinary team
• Implement therapeutic interventions to improve daily living skills, functional independence, and safety
• Recommend, fit, and train patients in use of adaptive equipment and assistive devices
• Provide direct instruction and supervised practice of activities of daily living (ADLs) and instrumental ADLs
• Educate patients and families on condition management and strategies to maximize independence
• Monitor, document, and adjust treatment plans based on patient progress and outcomes
• Coordinate with multidisciplinary team members to ensure comprehensive care and smooth community/workplace reintegration
• Maintain accurate, timely documentation including evaluations, treatment notes, and discharge planning
• Stay current with best practices and participate in required professional development
• Perform other duties as assigned by management
