Required qualifications

• Active RN license (WI or compact as applicable)
• Skilled nursing or long-term care experience preferred
• Strong clinical assessment, medication administration, documentation, and communication skills

Key responsibilities

• Provide and coordinate direct patient care in a skilled nursing/long-term care setting
• Administer medications and treatments per provider orders and facility protocols
• Monitor patient status, perform clinical assessments, and document care accurately and timely
• Participate in care planning and collaborate with interdisciplinary team members
• Maintain compliance with facility policies, state regulations, and documentation standards
