Contract Social Worker (LCSW Preferred) — Scarborough, ME — 14-Week Travel Assignment

Schedule: 07:00 AM–03:30 PM
Rate: Travel $1,032/24-hrs; Local $1,018/24-hrs

Minimum requirements:

• Bachelor’s degree in Social Work or Human Services
• State-required licensure/certification
• 1–3 years supervised social work experience in a direct healthcare setting (preferred)
• Additional certifications preferred (e.g., Geriatric Case Management, Hospice & Palliative Care, Clinical Social Work, Substance Use)

Key responsibilities:

• Plan and implement a comprehensive Social Services program in accordance with federal and state regulations
• Apply departmental policies and participate in Quality Assurance/Performance Improvement activities
• Complete timely, accurate documentation: psychosocial assessments, care plans, progress notes, discharge plans
• Conduct comprehensive psychosocial assessments; screen for trauma, mental illness, intellectual disability
• Provide therapeutic interventions and facilitate adjustment for patients/residents
• Lead and facilitate group sessions for patients/residents and families; conduct family meetings
• Protect and promote patient/resident rights and psychosocial well-being; identify and report abuse; participate in grievance investigations
• Coordinate and collaborate on discharge planning and transfers; communicate with patients/residents, families, staff, and community providers
• Provide staff support and interventions during catastrophic events
• Educate staff on the role of Social Services and patients’ psychosocial needs
• Supervise Social Service students as needed
• Perform other duties as assigned by management
