Licensed Social Worker (Contract/Travel) — Camden, ME (14-Week)

Join a contract Social Services team in Camden, ME (ASAP start, 14-week assignment, 08:00–16:00). Competitive pay: Travel $1,720/40-hrs; Local $1,696/40-hrs.

Minimum qualifications

• Bachelor’s degree in Social Work or Human Services.
• Current state-required certifications or licensure.
• 1–3 years’ supervised social work experience in a direct healthcare setting preferred.
• Additional certifications desirable (e.g., Geriatric Case Management, Hospice & Palliative Care, Clinical Social Work, Substance Use).

Key responsibilities

• Plan, implement and evaluate a comprehensive Social Services program that aligns with organizational goals.
• Apply departmental policies and procedures to ensure compliance with federal and state regulations.
• Participate in Quality Assurance and Performance Improvement activities.
• Maintain accurate, timely documentation including psychosocial assessments, individualized care plans, progress notes and discharge summaries.
• Safeguard patients’/residents’ rights and psychosocial welfare; identify, report and follow up on abuse and grievances.
• Conduct comprehensive psychosocial assessments, screen for trauma, mental illness and intellectual disability, and provide appropriate discharge planning.
• Deliver therapeutic interventions and facilitate support groups for patients/residents and families.
• Coordinate internal transfers, family meetings and multidisciplinary discharge planning; communicate effectively with patients, families, staff and community providers.
• Provide crisis and catastrophic-event interventions and staff support when needed.
• Educate staff on the role of Social Services and patients’ psychosocial needs; may supervise Social Service students.
• Perform other duties as assigned by management.
