* Day call Monday - Friday
* Day call weekend rotation
* 15 to 20 patients per day
* No primary night call
* Drive high-quality, safe, and cost-effective patient care in post-acute care communities
* Inpatient rounding in skilled nursing facility, inpatient rehabilitation facility, and long-term care hospital
* Collaborate with other clinician interdisciplinary teams and follow CMS guidelines
* Work closely with hospice, palliative care, and home care agencies for safe patient transitions
* Ensure timely documentation and billing through our EMR (Gehrimed) meeting Medicare guidelines

**Compensation & Benefits**

* Compensation is fee for service: expected compensation is estimated range of $252,000 to $302,000 annually with no cap on productivity income potential
