NP opportunity for Annual Health Assessments within Victorville, CA

Great opportunity within Victorville, CA for a Family Nurse Practitioner to perform annual wellness exams working for a private group. 

• Board Certified Family Nurse Practitioner (FNP)
• 1+ yrs exp as NP required - Primary Care, Family Med, Internal Med, Gerontology, or Palliative Care exp. Home Care exp is a plus
• Perform an annual wellness exam for Adult-Geriatrics, Medi-Cal, and PAVE patients - seeing all ages
• Collaborate with Primary Care team to ensure completion of holistic patient care
• This role is a mix of outpatient in-clinic setting, in-home health visits, and occasional telehealth appointments
• Must have active CA license, active DEA, currently enrolled in Medicare and in good standing, have HCC coding exp, and have AWV exp.
• Mon-Fri 8a-5p
• 8-10 patients per day
• Salary based on experience $145-170K with a $10K signing bonus or relo, plenty of great benefits and more
