• Monday - Friday full time
• 15 patients per day in group homes, 8-10 in individual homes
• Telehealth option with 15-20 patients
• Adult patients 18+, mostly geriatric population
• Patients with various disabilities
• Primary care focus with medication management
• Wound care experience with skin substitutes preferred
