Key Responsibilities

• Monitor patient status and coordinate follow-up with primary care providers and specialists
• Provide patient and family education on preventive care, chronic disease management, and treatment plans
• Maintain accurate documentation and meet quality metric and regulatory requirements
• Collaborate with multidisciplinary teams to optimize patient outcomes
