Facility: Outpatient Clinic within a non-profit Critical Access Hospital
Setting: Outpatient rheumatology clinic
Schedule: Monday–Friday, daytime clinic hours
Type: Permanent

Position summary
Provide independent outpatient care for adult patients with chronic and complex rheumatologic conditions. Work collaboratively with a rheumatology physician, nursing staff, and on-site Infusion Therapy Center.

Clinical duties

• Diagnose and manage chronic rheumatologic diseases in an outpatient setting
• Perform consults and follow-up visits for established rheumatology panel
• Coordinate care with Infusion Therapy Center and multidisciplinary team
• Provide patient education and longitudinal management
• Maintain accurate, timely clinical documentation in the facility EMR

Team and operations

• Reports to Rheumatology physician and clinical leadership
• Dedicated rheumatology physician backup
• Experienced nursing support and on-site infusion team
• No inpatient admissions or rounding duties

Qualifications

• NP (or PA) certification in good standing; certified Nurse Practitioner or Physician Assistant required
• Active Idaho license or eligibility to obtain one
• Prior rheumatology experience required
• Active or eligible DEA registration
• BLS required
• Ability to manage independent outpatient panel and collaborate in a team-based environment
• Visa sponsorship not available
