What you’ll bring

• Active RN license
• Experience in diabetes education or case management preferred
• CDCES preferred, or willingness to pursue certification
• Current BLS certification
• Strong skills in patient education, care coordination, organization, and communication

What you’ll do

• Provide personalized diabetes education and self-management training to patients
• Assess clinical needs and develop practical, patient-centered care plans
• Track blood glucose trends and support adherence to treatment plans
• Work closely with providers and interdisciplinary teams to coordinate care
• Teach patients about medications, insulin use, nutrition, and lifestyle strategies
• Accurately document progress, outcomes, and care plans
• Contribute to population health and chronic disease management efforts
