Registered Nurse (RN) — Home Health | Contract/Travel — Newport, VT (Mon–Fri Days)

Join a home health team in Newport, VT, on a contract/travel assignment starting ASAP. This is a day shift role (Mon–Fri, 8:00 a.m.–5:00 p.m.), offering $1,715 for a 32-hour week.

What you’ll be doing:

• Visit patients in their homes to assess health status, create individualized care plans, and track outcomes.
• Provide hands-on nursing care such as medication administration, wound care, and chronic disease management.
• Teach patients and families about their conditions, medications, lifestyle changes, and self-care techniques to support independence.
• Monitor and document progress, notify physicians of changes, and coordinate with the interdisciplinary team to ensure seamless care.
• Follow infection control, safety protocols, and regulatory requirements.
• Offer emotional support and connect patients and families with home-care resources.
• Contribute to quality improvement efforts and take part in ongoing professional development.

What we’re looking for:

• Current RN license in the state of practice.
• 1–2 years of nursing experience; home health experience is a plus.
• Strong clinical assessment, critical thinking, and communication skills.
• Ability to work independently and manage multiple patient visits efficiently.
• Valid driver’s license and reliable transportation.
• BLS certification required; additional certifications are a plus.
