You will assess patient needs, create and adjust individualized care plans, and coordinate care across multidisciplinary teams and hospital departments. Monitor patient progress, provide education to patients and families on treatment and discharge instructions, and maintain accurate clinical documentation. Ensure compliance with regulatory standards and quality metrics while supporting smooth transitions of care.

Requirements:

• Active RN license in the practicing state.
• Experience in inpatient nursing, case management, or care coordination preferred.
• Strong clinical judgment, organizational ability, and communication skills.
• BLS required; CCM or other care coordination certifications preferred.
