LPN/LVN – Long-Term Care, PM Shift (Middletown, RI) | 9-Week Contract, $1,464/40-hr

Requirements:

• Active, valid Licensed Practical Nurse (LPN) state license
• BLS certification

Core Responsibilities:

• Provide direct nursing care under RN/physician direction: medication administration, wound care, vital signs, and basic assessments.
• Perform focused patient assessments (physical exam, medical history, medication review) and participate in individualized care planning.
• Administer prescribed medications and treatments accurately and per protocol.
• Assess, clean, dress, and monitor wounds/incisions for infection or complications.
• Monitor and document vital signs (BP, pulse, temperature, respirations) and patient condition changes.
• Maintain accurate, timely patient records of assessments, treatments, and medication administration.
• Educate patients and families on conditions, medications, and self-care.
• Administer and monitor IV medications/fluids and maintain IV lines as indicated.
• Provide catheter care, including insertion maintenance and monitoring for complications.
• Collect specimens (blood, urine, sputum) for lab testing.
• Collaborate with the interdisciplinary team (RNs, physicians, therapists, support staff) to coordinate care.
• Address patient comfort and safety: positioning, mobility assistance, and pain management.
• Respond to emergencies following protocols and provide basic life support as needed.
• Follow infection control procedures and proper PPE use.
• Handle, store, and dispose of medications and supplies per regulations.
• Promote preventive care (vaccinations, screenings) and participate in health assessments.
• Advocate for patient rights and needs.
• May supervise CNAs or unlicensed assistive personnel when assigned.
• Complete other duties as assigned by management.
