Requirements:

• Active Licensed Practical Nurse (LPN) state license
• BLS certification

Key Responsibilities:

• Provide direct patient care under RN/physician direction, including medication administration, wound care, IV therapy, catheter care, and vital signs monitoring.
• Conduct focused patient assessments (physical exam, history, medication review) and implement individualized care plans.
• Administer medications and treatments accurately, following protocols and documenting administration.
• Assess, clean, dress, and monitor wounds/incisions for complications or infection.
• Insert, monitor, and maintain IV lines and urinary catheters; monitor for adverse events.
• Collect specimens (blood, urine, sputum) for laboratory testing.
• Monitor, record, and report changes in patient condition and vital signs; maintain accurate patient records.
• Provide patient and family education on conditions, treatments, medications, and self-care.
• Assist with patient comfort, mobility, positioning, and pain management.
• Respond to emergencies per protocols and provide basic life support as needed.
• Follow infection control practices and proper handling, storage, and disposal of medications and supplies.
• Collaborate with the interdisciplinary team (RNs, physicians, therapists, support staff) to coordinate care.
• Supervise CNAs or unlicensed assistive personnel when assigned.
• Maintain professional competence through ongoing education.
• Perform other duties as assigned by management.
