Minimum requirements:

• Active, unrestricted RN license for practice in the state
• BLS certification; ACLS may be required by facility
• Experience in post-acute, sub-acute rehab, skilled nursing, or long-term care preferred
• Strong assessment, clinical judgment, critical thinking, prioritization, and multitasking skills
• Knowledge of state/federal LTC regulations and infection control standards
• Excellent communication and documentation skills
• Patient-centered, compassionate approach

Primary responsibilities:

• Perform initial and ongoing nursing assessments; document resident condition accurately
• Develop, implement, and update individualized care plans with the care team
• Administer medications and treatments per physician orders and facility protocols
• Monitor residents for condition changes and initiate appropriate interventions
• Provide wound care, IV therapy, and post-acute rehabilitation support as required
• Coordinate care with physicians, therapists, CNAs, and other staff to ensure continuity
• Educate residents and families about care plans, medications, and health management
• Maintain accurate, timely, and regulatory-compliant charting (including MDS input as applicable)
• Assist with admissions, transfers, and discharges
• Enforce infection control practices and regulatory compliance
• Respond to emergencies and perform life-saving measures when necessary
• Supervise and support CNAs and support staff to maintain quality standards
