**Advanced Practice Provider – Internal Medicine**
**Available Locations: Southbury / Naugatuck / Shelton**
Opportunity Details
• Full-time Nurse Practitioner (NP) or Physician Assistant (PA) role
• Support internal medicine services across outpatient and hospital settings
• Perform history & physicals, assist with inpatient and outpatient consultations
• Collaborate with physicians to develop and implement individualized care plans
• Comprehensive management of patients, including physiologic, social, and psychological assessment
• Support care coordination and continuity for complex patient populations
• Involvement in patient education, preventive care, and chronic disease management
Requirements
• Active Connecticut APRN or PA license
• Connecticut Controlled Substance Registration and DEA
• Experience in internal medicine or primary care required
• Strong clinical, organizational, and communication skills
