Position Summary

Horizon Family Medical Group is seeking a Full-Time Nurse Practitioner to join our team in a hybrid Pulmonology and Primary Care role. This position offers a unique opportunity to provide both general primary care and specialized pulmonary services within a supportive, well-established multi-specialty practice.

The ideal candidate will be a compassionate, patient-focused clinician who enjoys variety in their day-to-day work—balancing primary care responsibilities with pulmonary consultations and follow-ups. You’ll work closely with physicians, nurses, and clinical staff to ensure seamless, high-quality care for every patient.

This role combines clinical independence with strong team collaboration, supported by a network that values innovation, professional growth, and work-life balance.

Key Responsibilities

  • Provide comprehensive primary care and pulmonary services, including preventive care, chronic disease management, and acute visits.
  • Conduct patient assessments, order and interpret diagnostic tests, and develop individualized treatment plans.
  • Manage and follow up on pulmonary patients, including those with chronic respiratory conditions.
  • Collaborate with supervising physicians and multidisciplinary teams to optimize patient outcomes.
  • Maintain accurate, timely documentation within the EMR—with optional AI or virtual scribe support.
  • Educate patients on wellness, disease prevention, and treatment plans.
  • Travel as needed to the New Windsor, NY office for scheduled hours.

Qualifications

  • Graduate of an accredited Nurse Practitioner program.
  • Current and active New York State NP license in good standing.
  • Minimum 1 year of experience in PulmonologyPrimary Care, Critical Care, Internal Medicine, or .
  • Strong communication, clinical judgment, and interpersonal skills.
  • Commitment to patient-centered, compassionate care.
  • Excellent attention to detail, organization, and problem-solving abilities.
