• Team-based care with IDT meetings twice weekly in afternoons
• 3 - 9 patients per day with extended one-on-one time
• Monday - Friday with participants completed by 3pm
• May travel to assisted living and skilled nursing facilities
• 1:4 call schedule primarily telephone-based
• Family medicine, geriatrics, or primary care experience required
• Hospital privileges required
• BLS certification required
