Rate: Travel $2,308 / 36 hrs; Local $2,275 / 36 hrs

Requirements:

• Valid, active Occupational Therapy state license
• CPR and BLS certification from AHA or Red Cross (with AED)

Core Responsibilities:

• Evaluate patients’ physical and cognitive abilities and home/environmental factors to identify limitations and establish goals
• Develop individualized treatment plans in collaboration with patients and interdisciplinary team members
• Implement therapeutic interventions to improve daily living skills, independence, and functional performance
• Recommend, train in, and fit adaptive equipment and assistive devices
• Teach and rehearse activities of daily living (ADLs) and instrumental ADLs with patients
• Educate patients and caregivers on condition management and independence strategies
• Monitor, document, and revise treatment plans based on patient progress and outcomes
• Maintain accurate clinical documentation, including evaluations, treatment notes, and discharge plans
• Coordinate care with multidisciplinary team to support safe return to home, community, and work environments
• Participate in ongoing professional development and stay current with OT best practices
• Perform other duties as assigned by management
