- Monday through Thursday 8 am - 5 pm with possible admin day
- 16 patients per day
- Ages 6 and up
- EMR: eClinicalWorks
- Chronic care management focus
- COPD, hypertension, cholesterol, diabetes management
- Hospital privileges required
