Position Overview

Serve patients in moments that matter and be a part of the future of health care. SCP Health is seeking a dedicated Nurse Practitioner to join our dynamic inpatient rehab team at CHRISTUS St. Michael Hospital. Join us to grow your career, make a meaningful impact, and deliver high-quality care in the communities you serve. Working hours are from 8 am to 5 pm Monday through Friday. There is no overnight call or responsibilities after 5 pm and no weekend call or duties.


Daily duties

1. Consulting on all new admits from the previous day. You determine if and when they need to be seen again. (approximately 3 new admits a day)

2. Rounding on the remaining patients that you have previously seen and determined need to be seen that day. I would anticipate this list being around 15 patients. They have about 40 patients there any day and likely some will not need to be seen at all and the rest at most would likely need to be seen no more frequently than every other day or every three days. There may be the exception that someone will need to be seen on consecutive days but that would be at your digression.

3. Answering any new questions or issues should they arise on patients previously seen and that may need to be seen again.

4. Answering all calls and concerns regarding patients from 8 am to 5 pm Monday through Friday.

5. Attending our monthly staff meeting in person or via phone on the fourth Thursday of every month (It is at the main hospital).

6. Attending a monthly meeting at rehab where patient care and issues related to their care will be discussed.

7. Ordering labs, studies and imaging as indicated and following up on those results.

8. Helping to form a strong bond between rehab and the main hospital via two way communication with the hospitalist group (CS Partners) regarding patients we have sent there and any other questions that may arise while seeing patients at the facility.

9. Calling to arrange for direct admission for patients that need to be sent back to the hospital for admission.

10. Working collaboratively with the rehab providers, nursing and ancillary staff to provide optimal patient care.

11. Entering all of your billing daily before your shift ends.

12. Managing and updating your patient lists in our SCP connect platform and EPIC (hospital and rehab EMR) 



Qualifications

  • Education: PA
  • Licensure: TX
  • Board Certification: Yes
  • Experience: Minimal of 2 years of PA inpatient experience
