Travel RN — Home Health (St. Johnsbury, VT) | $2,434/36-hr, Mon–Fri Days, ASAP

We’re seeking a dedicated Registered Nurse to join a home health team in St. Johnsbury, VT on a contract/travel basis. This role is Monday–Friday, daytime hours (36-hour schedule) with a competitive pay rate of $2,434 per 36 hours. Start date: ASAP.

What you’ll do

• Visit patients in their homes to assess health status, develop individualized care plans, and evaluate outcomes.
• Deliver nursing interventions such as medication administration, wound care, and chronic disease management.
• Teach patients and families about disease management, medications, lifestyle changes, and self-care techniques.
• Monitor and document patient progress, report changes to physicians, and coordinate care with the interdisciplinary team.
• Follow infection control, safety protocols, and regulatory standards.
• Provide emotional support and practical guidance on home care resources.
• Take part in quality improvement efforts and ongoing professional development.

We’re looking for someone who has

• A current RN license valid in the state of practice.
• 1–2 years of nursing experience (home health experience is preferred).
• Strong clinical assessment, critical thinking, and communication skills.
• Comfort working independently and managing a schedule of multiple patient visits.
• A valid driver’s license and reliable transportation.
• Basic Life Support (BLS) certification; additional certifications are a plus.
