Requirements:

• Active RN license (WI or compact as applicable)
• Experience in skilled nursing or long-term care preferred
• Strong clinical assessment, medication administration, documentation, and communication skills

Core responsibilities:

• Provide and coordinate direct patient care in a skilled nursing/long-term care setting
• Administer medications and treatments per orders; document administration and patient response
• Monitor and assess patient conditions; report changes and update care plans
• Collaborate with interdisciplinary team members to support care planning and continuity
• Maintain compliance with facility policies, infection control standards, and regulatory requirements
