Contract/Travel RN On-Call — Hospice (Fort Washington, PA)

Rate: Travel $771/32 hrs; Local $739/32 hrs

Requirements:

• Active Registered Nurse license
• Hospice care knowledge; familiarity with Medicare/Medicaid guidelines
• Strong assessment, documentation, and communication skills
• Ability to work independently in home-care settings
• IV therapy and venipuncture experience preferred
• Valid driver’s license, reliable vehicle, and insurance
• Ability to meet physical demands of home-based care
• Commitment to hospice philosophy and ethical nursing standards

Key Responsibilities:

• Respond to after-hours patient/family calls; assess needs and triage appropriately
• Provide pain and symptom management per physician’s plan of care
• Perform and document ongoing patient assessments accurately and timely
• Communicate patient status and changes to physicians and the interdisciplinary team
• Educate and support patients and caregivers regarding care and symptom management
• Coordinate referrals to hospice disciplines and community resources as needed
• Maintain patient confidentiality and comply with agency policies and procedures
• Work independently, prioritize urgent situations, and escalate per protocol
