Requirements:
- Active Registered Nurse license
- Knowledge of hospice care, Medicare, and Medicaid guidelines.
- Strong assessment, documentation, and communication skills.
- Ability to work independently in home-care environments.
- Experience with IV therapy and venipuncture preferred.
- Valid driver’s license, reliable vehicle, and insurance.
- Ability to meet physical demands of home-based patient care.
- Commitment to hospice philosophy and ethical nursing standards.
Responsibilities:
- Respond to after-hours patient/family calls and assess needs.
- Provide pain and symptom management per physician plan of care.
- Perform ongoing patient assessments and document care accurately.
- Communicate with physicians and interdisciplinary team members.
- Educate and support patients and caregivers.
- Coordinate referrals to hospice disciplines and community resources.
- Maintain confidentiality and comply with agency policies.
- Work independently and prioritize urgent situations.
