Rate: Travel: $771/32 hrs | Local: $739/32 hrs

Requirements:

• Active Registered Nurse license
• Knowledge of hospice care and Medicare/Medicaid guidelines
• Strong assessment, documentation, and communication skills
• Ability to work independently in home-care settings
• IV therapy and venipuncture experience preferred
• Valid driver’s license, reliable vehicle, and insurance
• Ability to meet physical demands of home-based care
• Commitment to hospice philosophy and ethical nursing practice

Responsibilities:

• Respond to after-hours patient/family calls and perform needs assessments
• Provide pain and symptom management per physician orders
• Conduct ongoing patient assessments and document care accurately
• Communicate clinical findings and status to physicians and the interdisciplinary team
• Educate and support patients and caregivers regarding care plans and symptom management
• Coordinate referrals to hospice disciplines and community resources as needed
• Maintain patient confidentiality and comply with agency policies
• Work independently and prioritize urgent situations in the field
