Schedule: Sun–Sat, 08:00 AM–04:30 PM
Rate: Travel $2,317/35 hrs; Local $2,282/35 hrs

Requirements:

• Active, valid Occupational Therapy state license
• CPR and BLS certification (AHA or Red Cross, including AED)

Key Responsibilities:

• Assess patients’ physical and cognitive abilities and evaluate home/environmental barriers
• Establish functional goals and develop individualized treatment plans in collaboration with patients and the care team
• Implement therapeutic interventions to improve activities of daily living (ADLs), independence, and community/work reintegration
• Recommend, fit, and instruct on adaptive equipment and assistive devices
• Train and practice daily living tasks with patients to build skills and safety
• Educate patients and families on condition management and strategies to maximize independence
• Monitor, document, and revise treatment progress, including evaluations and progress notes
• Communicate and coordinate care with a multidisciplinary team
• Maintain professional competency through continuing education
• Perform other duties as assigned by management
