Rate: Travel: $2,308/36 hrs; Local: $2,275/36 hrs

Requirements:

• Active Occupational Therapy state license
• CPR and BLS certification (AHA or Red Cross, including AED)

Key Responsibilities:

• Assess patients’ physical, cognitive, and environmental limitations; establish measurable goals.
• Develop individualized treatment plans in collaboration with patients and the interdisciplinary team.
• Implement therapeutic interventions to improve activities of daily living (ADLs), instrumental ADLs, cognition, and independence.
• Recommend, fit, and train patients in the use of adaptive equipment and assistive devices.
• Provide hands-on instruction and practice for daily living tasks.
• Educate patients and families on condition management and strategies to maximize independence.
• Monitor, document, and revise treatment plans based on patient progress; maintain accurate evaluations and progress notes.
• Collaborate with multidisciplinary staff to coordinate care and facilitate return to community and work.
• Stay current with best practices and engage in professional development.
• Perform other duties as assigned by management.
