Required credentials and experience:

• Active RN license (ME or compact as applicable)
• Graduate of an accredited nursing program (BSN preferred)
• BLS certification
• Previous case management or acute care experience preferred
• Knowledge of discharge planning and insurance/utilization guidelines
• Strong communication, organizational, and problem‑solving skills

Primary responsibilities:

• Assess patient needs and develop individualized care plans
• Coordinate care across providers, services, and care settings
• Facilitate discharge planning and transitions of care
• Collaborate with interdisciplinary teams and community resources
• Monitor patient progress and revise plans as needed
• Ensure appropriate utilization of services and resources
• Educate patients and families on treatment plans and follow‑up care
• Document care coordination activities accurately in the EMR
