Travel RN – Home Health (Rutland, VT) – Days, Mon-Fri, 37 hrs/week

Join a dedicated home health team in Rutland, VT (contract/travel). This daytime, Monday–Friday role (8:00 a.m.–4:00 p.m.) requires a motivated Registered Nurse to deliver high-quality, patient-centered care across home visits. Compensation: $2,501 per 37-hour week. Start: ASAP.

Requirements:

• Active RN license in Vermont.
• 1–2 years nursing experience; home health experience preferred.
• Strong clinical assessment, critical thinking, documentation, and communication skills.
• Ability to work independently, prioritize, and manage multiple patient visits efficiently.
• Valid driver’s license and reliable transportation.
• BLS certification required; additional certifications a plus.

Key responsibilities:

• Perform comprehensive patient assessments, develop individualized care plans, and evaluate outcomes.
• Deliver nursing interventions including medication administration, wound care, and chronic disease management.
• Educate patients and families on diagnoses, medications, lifestyle modifications, and self-care techniques.
• Monitor and document patient progress, report changes to physicians, and coordinate care with interdisciplinary teams.
• Adhere to infection control, safety protocols, and regulatory standards.
• Provide emotional support and guidance on home care resources.
• Participate in quality improvement efforts and ongoing professional development.
