What you’ll do:

• Provide hands-on assistance with activities of daily living and routinely assess resident condition.
• Safely administer medications and treatments per RN or physician direction.
• Accurately document care, communicate changes in status, and coordinate with the nursing team.
• Maintain a clean, safe environment and uphold facility policies.
• Offer compassionate emotional support and companionship to residents.

Qualifications:

• Active LPN/LVN license in the practicing state.
• Long-term care, skilled nursing, or assisted living experience preferred.
• Strong clinical judgment, clear communication, and reliable caregiving skills.
• Able to follow care plans and collaborate effectively with nursing staff.
