Required credentials

• Active Occupational Therapy state license
• CPR and BLS certification from American Heart Association or Red Cross (with AED)

Primary responsibilities

• Evaluate patients’ physical, cognitive, and environmental needs; identify functional limitations and set measurable goals.
• Develop and implement individualized treatment plans in collaboration with patients and the interdisciplinary team.
• Use therapeutic techniques to improve activities of daily living, functional mobility, cognition, and independence.
• Recommend, fit, and train patients on adaptive equipment and assistive devices.
• Provide task-specific training and education to patients and caregivers for condition management and independence.
• Monitor, document, and modify treatment based on patient progress and outcomes.
• Maintain accurate clinical records including evaluations, treatment plans, and progress notes.
• Coordinate with physicians, nurses, therapists, and other providers to support discharge planning and community/work reintegration.
• Participate in continuing education and maintain competence in evidence-based OT practices.
• Perform other duties as assigned by management.
