Rate: Travel: $1,775/35-hours; Local: $1,750/35-hours

Requirements:

• Valid, active Occupational Therapy state license
• CPR and BLS certification from American Heart Association (AHA) or Red Cross (including AED)

Responsibilities:

• Evaluate patients’ physical and cognitive abilities and environments to identify limitations and set measurable goals
• Develop individualized treatment plans in collaboration with patients and the interdisciplinary team
• Implement therapeutic interventions to improve activities of daily living (ADLs), independence, and functional skills
• Recommend and instruct on adaptive equipment and assistive devices
• Train and coach patients in daily living tasks to build skills and independence
• Educate patients and families on condition management and strategies to increase autonomy
• Monitor, document, and revise treatment plans based on patient progress and outcomes
• Maintain accurate patient records, including evaluations, treatment notes, and discharge summaries
• Coordinate care with multidisciplinary team members to support safe discharge and community/work reintegration
• Participate in ongoing professional development to remain current with best practices
• Perform other duties as assigned by management
