Requirements

• Valid, active Occupational Therapy state license
• CPR and BLS certification from American Heart Association or Red Cross (with AED)

Responsibilities

• Evaluate patients’ physical and cognitive abilities and home/environmental factors to identify limitations and set measurable goals
• Develop individualized treatment plans in collaboration with patients and the healthcare team
• Implement therapeutic interventions to improve activities of daily living (ADLs), instrumental ADLs, motor function, cognition, and independence
• Provide training and recommendations for adaptive equipment, assistive devices, and environmental modifications
• Instruct patients and caregivers in daily living tasks and safe techniques; practice skills with patients to build independence
• Educate patients and families on condition management and strategies to promote independence
• Monitor, document, and revise treatment plans based on patient progress; maintain accurate evaluations, treatment notes, and discharge summaries
• Coordinate care with a multidisciplinary team to support safe community and workplace reintegration
• Maintain professional competence through ongoing education and adhere to facility policies and procedures
• Perform other duties as assigned by management
