• Monday - Friday 8 am - 5 pm (9-hour shifts)
• 18 patients per shift minimum
• FQHC experience required
• Pediatric chronic care management experience required
• Behavioral health anxiety treatment experience required
• Ongoing assignment
• Focus on chronic care and behavioral health
• 5 years similar setting experience preferred
