- Monday - Friday 8 am - 5 pm shifts
- 18 patients per shift minimum
- FQHC setting
- Pediatric chronic care management and behavioral health focus
- 5 years FQHC experience preferred
- Anxiety treatment experience required
- Ongoing assignment length
