Required Qualifications
- Active LPN/LVN license in the practicing state
- Long-term care, skilled nursing, or assisted living experience preferred
- Competent in medication administration and basic nursing treatments
- Strong clinical assessment, documentation, communication, and caregiving skills
- Ability to follow care plans and collaborate with RNs and the interdisciplinary team
Primary Responsibilities
- Assist residents with activities of daily living (ADLs)
- Monitor and assess resident health status; report changes promptly
- Administer medications and treatments per facility policy and under RN/physician oversight
- Accurately document care, observations, and interventions
- Maintain a safe, clean, and supportive environment
- Provide emotional support and companionship to residents
