We’re looking for an experienced LPN to join the care team in Clayton for a 13-week contract. You’ll work evenings and provide hands-on nursing care while collaborating with RNs, physicians, therapists, and support staff to keep patients safe, comfortable, and progressing toward their goals.

What you’ll do:

• Provide direct nursing care under RN or physician direction — medication administration, wound care, vital signs, and more.
• Perform patient assessments (physical exams, histories, medication reviews) to help develop individualized care plans.
• Give prescribed medications and treatments accurately and follow facility protocols.
• Clean, dress, and monitor wounds and surgical sites for signs of infection or complications.
• Regularly monitor and document vital signs: blood pressure, pulse, temperature, and respirations.
• Keep clear, accurate patient records that reflect changes in condition and treatments given.
• Teach patients and families about diagnoses, medications, self-care, and wellness strategies.
• Administer and observe IV medications and fluids when required, ensuring lines are functioning properly.
• Care for patients with urinary catheters—insert when needed, perform routine maintenance, and watch for issues.
• Collect samples (blood, urine, sputum) for lab testing.
• Assist with patient comfort and safety needs: repositioning, mobility help, and pain management.
• Respond quickly and appropriately in emergencies, providing basic life support as needed.
• Follow strict infection control practices, including hand hygiene and appropriate PPE use.
• Handle, store, and dispose of medications and supplies safely and according to guidelines.
• Promote preventive care (vaccinations, screenings) and support health assessments.
• Advocate for patients’ rights and ensure they receive the care and resources they need.
• Stay current with nursing best practices and complete required training.
• Supervise CNAs or unlicensed assistive personnel when applicable.
• Perform other duties as assigned by management.

Requirements:

• Active, valid Licensed Practical Nurse state license
• BLS certification
