Requirements

  • Active, valid Licensed Practical Nurse (LPN) state license
  • BLS certification

Core Responsibilities

  • Provide direct nursing care under RN/physician guidance, including medication administration, wound care, IV therapy, catheter care, and vital sign monitoring.
  • Perform focused patient assessments (physical exam, medical history, medication review) to implement individualized care plans.
  • Administer prescribed medications and treatments accurately, following protocols and proper documentation.
  • Assess, clean, dress, and monitor wounds/incisions for signs of infection or complications.
  • Monitor and document vital signs and changes in patient condition; report significant findings to the supervising RN/physician.
  • Collect specimens (blood, urine, sputum) for laboratory testing.
  • Maintain accurate, timely patient records, including treatments, medications, and status changes.
  • Provide patient and family education on diagnoses, medications, self-care, and preventive measures.
  • Assist with positioning, mobility, comfort measures, and basic pain management.
  • Respond to medical emergencies per facility protocols and provide BLS as needed.
  • Follow infection control practices and proper use of PPE.
  • Safely handle, store, and dispose of medications and medical supplies in accordance with regulations.
  • Collaborate with interdisciplinary team members (RNs, physicians, therapists, support staff) to coordinate care.
  • May supervise CNAs or unlicensed assistive personnel when assigned.
  • Perform additional duties as assigned by management.
