Responsibilities:
- Guide patients through the cancer care continuum
- Coordinate appointments, diagnostics, and treatments
- Provide education on diagnosis, treatment options, and side effects
- Serve as liaison between patients, providers, and care teams
- Identify barriers to care and connect patients with resources
- Monitor patient progress and support care coordination
- Document navigation activities and patient interactions in EMR
